While running a staff development program we discovered that each of the 10 employees had different interpretations of the organization chart and who reported to whom!  We also found that job duties overlapped so much no one took responsibility for them! And worse yet, we discovered so much duplication in work we could  reduce staff by five!  Can't happen at your school?  It probably does.
